Privacy clause for forms

The personal information requested on this form is being collected by Port Phillip City Council for the purpose of understanding the views of young adults for the What’s Your Story?: an alcohol culture change initiative.  The personal information will be used solely by the council for that primary purpose or directly related purposes. Council may disclose this information to our partners who are directly involved in the project including VicHealth, The City of Stonnington and the City of Melbourne.  Unidentified data will be shared with our evaluation partners Monash University and La Trobe University.  If this information is not collected we will know less about the important views and experiences of young adults who enjoy late night entertainment precincts.  The applicant understands that the personal information provided is for the What’s Your Story?: an alcohol culture change initiative and that he or she may apply to the council for access to and/or amendment of the information. Requests for access and or correction should be made to Council’s Information Privacy Officer.
